The application is not intended as a substitute for the services of a medical professional.

We operate the HEALAX web and mobile applications and the HEALAX Services. Our policies regarding the collection, use, and disclosure of Personal Information when you use our Services are set forth herein and in the Terms of Service Agreement (wherein this Privacy Policy is incorporated by reference). We will not use or share your information with anyone except as described in this Privacy Policy or the Terms of Service Agreement. By using the Services, you agree to the collection and use of information in accordance with this policy.

While using our Services, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you. Personally identifiable information may include but is not limited to, your email address, name, address, phone number, feedback and survey responses, and other information and health-related information included within your messages to us (“Personal Information”). - With companies and contractors perform services for us. These third parties have access to your Personal Information only to perform specific tasks on our behalf and are obligated not to disclose or use it for any other purpose;
